Athlete consent

ARC HOCKEY — PARENT/GUARDIAN CONSENT AND VIDEO RELEASE Version 2026-09-05.v2 1. Who we are. ARC Hockey, Inc. ("ARC", "we") is a US company that produces goaltender and player development reports from video. This form is your consent for ARC to do that for the athlete named below (the "Athlete"). Questions: privacy@mygoaliecoach.ai. 2. What you are providing. Video of the Athlete's games and practices, filmed by you, the Athlete's coach, or ARC, together with the Athlete's name, date of birth, team and level, and your contact details. 3. What we do with it. We process the video with computer-vision and AI models to produce a written development report and performance scores for the Athlete. Video and derived data are stored and processed in the United States and are subject to US law while there. 4. Who sees the report. The report is provided to you (the parent/guardian) and to the ARC-registered coach you select below. ARC-registered coaches have signed ARC's coach terms, which limit their use of the report to coaching the Athlete and prohibit them from sharing it further without your permission. You may change or withdraw a coach designation at any time by emailing privacy@mygoaliecoach.ai. 5. Other people in the video. Game footage may show other players and spectators. ARC analyzes only the Athlete named below, does not produce reports on anyone else in the footage, and does not redistribute raw video. 6. Optional uses (only if you check the box). We will not do either of the following unless you opt in below: (a) use the Athlete's video and derived data to improve ARC's analysis models; (b) share the Athlete's report with ARC-registered scouts, programs, or leagues for recruiting purposes. 7. Retention and deletion. Video is retained for the current season plus 12 months, then deleted. Reports and scores are retained while the Athlete has an ARC profile. You may ask us to delete the Athlete's video and data at any time by emailing privacy@mygoaliecoach.ai; we will confirm deletion within 30 days, except where we are required to keep a record of this consent. 8. Security. Access to video and reports is restricted to ARC staff who need it and to the coach you designate. We use encrypted storage and transfer. If a security incident affects the Athlete's data we will notify you without undue delay. 9. Your rights. You may ask what personal information we hold about the Athlete, ask us to correct it, or withdraw this consent at any time. Withdrawal stops future processing; it does not undo analysis already delivered. Canadian residents may complain to the Office of the Privacy Commissioner of Canada; US residents may have rights under their state's privacy law. 10. Video release. You grant ARC a non-exclusive license to store and process the video you provide for the purposes in this form. ARC will not use the Athlete's name, image, or video in marketing without your separate written permission. 11. Your confirmation. By typing your name and checking the boxes below you confirm that you are the parent or legal guardian of the Athlete, that you have read and understood this form, and that you agree to it on the Athlete's behalf. Where the Athlete is 13 or older, we ask that they read this form too and acknowledge it alongside you. This form is governed by the laws of the State of North Carolina, without prejudice to any protections you have under the law of the place where you live.
